Every Passport issued by Her Majesty's Consuls should, like the Passports issued from the Foreign Office, bear, in some part of it, the Signature of the British subject to whom it is granted; and the Consul is instructed to require snch Signature in every case in which he may be called upon to grant a Passport.

B., Her Britannic Majesty's Vice-Consul (or British Vice-Consul) at (Town), on behalf of Her Majesty's Principal Secretary of State for Foreign Affairs, and by virtue of the authority in me vested, do hereby appoint (C. D.) to be British Pro-Consul at (Town), for the purpose of administering oaths, and taking affidavits or affirmations, and of performing all and every notarial acts and act under the powers conferred on Pro-Consuls by the Act 18 & 19 Victoria, cap. 42, and subject to the conditions laid down in regard to such appointment by a Circular Despatch of the Right Honourable the Earl of Derby to Her Majesty's Consular Representatives abroad, dated the 6th day of November, 1876. Information to be given by or on behalf of the Claimant.

The following Questions, &c., are to be answered in writing, according to the best of your information and belief :—

Where did he last live when in this country?

State what his occupation or employment was at the time of his death, and at any time previously?

Did he leave any will?

Has any body taken, or is any body intending to take out letters of administration of his estate?

State the names of his father and mother; and if they are dead say so, and give the date of death, if known.

State the name of the deceased's widow (if any), and say whether she is still alive. If the deceased was not married, insert " unmarried." State the names of all his children, including the husbands', if living, of any married daughters; if there are no children, say so.

State the names of the brothers and sisters (if any) of the deceased, including the husbands', if living, of any married sisters.

State the names of any other of the relations of the deceased.

If the Claimant or Informant possesses any letters from the deceased,
