(c) State the Federal corporate income tax paid for by your corporation for each year covering periods subject to this survey.

(d) State (if possible) the State corporate income tax paid by your corporation for each year covering periods subject to this survey.

2. Indicate at which corporate level or levels you purchased beef during the period covered by this survey:
