28. For each period covered by this survey, state the total expenses for repairs on equipment used for the beef slaughtering or the beef by-product/offal operations.

29. For each period covered by this survey, state the total expenses for uniforms incurred for the beef slaughtering and beef by-product/ offal operations.

30. For each period covered by this survey, state the total expenses for licenses required for the beef slaughtering and beef by-product/offal operations.

If, for any item 14 through 32, disaggregate data for the beef slaughtering and beef by-product/offal operations are not available, give the aggregate data for all slaughtering and by-product total operations, and identify the other items included.

5. TRANSPORTATION

For all transportation undertaken by the company by methods other than trucking, answer questions 32 through 41.

For all transportation undertaken by the company through trucking, answer questions 1 through 31.

1. For all procurement for and distribution from this slaughtering facility, is truck transportation undertaken by:

(a) A company-affiliated trucking fleet

(b) A non-affiliated trucking fleet

(c) Combination of the above

If the answer to 1 is (a), questions 2 through 27 apply to costs incurred for the company's trucking fleet.

If the answer to 1 is (b), answer questions 28 through 31 on transportation provided by non-affiliated firms.

If the answer to 1 is (c), answer all questions, 2 through 31.

AFFILIATED TRUCKING COMPANIES

2. State the total number of employees and the total payroll for the transportation operation on the first day of each period covered by this survey. Include transportation executives and their secretaries, truck drivers, etc.

3. For each period covered by this survey, state the total expenses for employee benefits incurred for the transportation operation. Employee benefits include social security, workmen's compensation, bonuses, insurance benefits, pension plans, profit sharing, etc.

4. For each period covered by this survey, state the total expenses for union welfare incurred for the transportation operation.

5. For each period covered by this survey, state the total expenses for travel and entertainment including hotel charges, meals, car

allowance or mileage, tolls, tips, and all other out-of-pocket expenses for individuals involved in the transportation operation.

6. For each business week covered by this survey, state the total number of pounds of non-meat items transported by company owned trucks and the total assessed cost of such transportation.

7. Does the company set a freight rate for intra-company transportation? If so, what was this rate for each period covered by this

survey.

PROCUREMENT TRUCKS

8. State the total book value (declared for Federal tax purposes) of the procurement truck fleet and other procurement related equipment on the last day of each fiscal year covering periods subject to this survey.

9. Indicate the amount of depreciation (as reported for Federal tax purposes) taken on the procurement truck fleet and other procurement transportation related equipment for each year covering periods subject to this survey. Indicate the date through which the depreciation is calculated.

10. For each period covered by this survey, state the percentage of the procurement trucking fleet and other procurement transportation equipment was (1) owned (2) rented or leased.
